They don't ask for anything except money. You can buy a 7 day visa for 10 US Dollar or a 30 day visa for 25 US Dollar. These are tourist visa, for any other type you have to go to the Indonesian Embassy at Tobias Asserlaan 8 in Den Haag (The Hague). They are actually quite helpful there.

You can't get visa on arrival (VOA) at all indonesian airports and seaports, but the ones most often used offer it (Jakarta, Surabaya, Denpasar, Medan, Menado). They payment methods differ per airport.

In Jakarta, you can pay with Visa or Mastercard, but even at Surabaya that was not possible. They did accept a combination of Indonesian Rupiah and Malaysian Ringgit. Dollars and Euro are also possible.

If you want to be on the safe side, bring US Dollars. Best options and best exchange rate. A couple of times I even went through immigrations, found and ATM, went back and paid the Visa in Rupiah. Just ask one of the officers.

Best to take US dollars for a visa on arrival (VOA), I had to use an ATM once as the Indonesian Rupiah I had in my wallet, although more than enough to cover the 25 US dollars to the current exchange rate on the day, was still not enough for the immigrations officers personal exchange rate.

When I returned to the VOA booth after using the ATM, the price in Rupiah had risen by about USD$5.00. I complained and the immigration officer nearly shit his pants and the price returned back down.  I had not even technically passed the Indonesian border and had my passport stamped yet and was subject to corruption!!!!!!!!!!!
